When an accident related to work tasks is reported, the relevant CRAMS is brought off the live system and dropped into ‘Pending Review’. This allows the Competent Person to determine whether it’s staff behaviour or the method statement itself at fault. The next step would be to re-train the staff member(s) in question or amend the method statement, with any change to the document resulting in all users being notified when they next log in.
